Representações sociais dos enfermeiros face ao idoso em contexto de prestação de cuidados

Abstract

O envelhecimento é um processo normal que não é bem compreendido, o que tem contribuído para o aparecimento de mitos e estereótipos relativamente aos idosos. O idoso deixou de ser visto, como um ser de sabedoria, um contador de histórias, mas sim como um “velho" que dá trabalho, está à espera da morte, e deixou de ser útil. Os idosos, são cada vez mais vítimas de discriminação e de estereótipos e há poucas políticas de apoio e protecção aos idosos e às suas famílias o que contribui para os isolar, para os afastar e fechar em instituições. Estes aspectos negativos, ilustram apenas certas dimensões do envelhecimento, contribuindo, para manter os estereótipos, mitos, e as ideias pré-concebidas. Neste sentido, propusemo-nos conhecer as Representações Sociais dos enfermeiros ao cuidar do idoso, para determinar a influência que as mesmas possam ter no acto de cuidar. Relativamente aos procedimentos metodológicos, optámos por um estudo de carácter descritivo, exploratório, inserido numa abordagem qualitativa, uma vez que permite, conhecer e compreender as experiências, atitudes, e opiniões dos enfermeiros do estudo no contexto de prestação de cuidados ao idoso. Realizámos entrevistas semi-directivas, a dezoito enfermeiros (Centro de Saúde e Hospital). A análise dos dados teve como quadro conceptual a teoria das Representações Sociais. Foram definidas três áreas temáticas: Conceito de idoso/velhice/envelhecimento; Vivências que os enfermeiros têm em relação ao idoso; Atitudes/opiniões dos enfermeiros face aos cuidados de enfermagem ao idoso. Na primeira e segunda área, é reconhecida a questão conceptual do que é ser idoso e de que forma as vivências dos enfermeiros podem influenciar esse conceito. Para a maioria é ser dependente, só, infeliz, abandonado, necessita de atenção, carinho, é crianças duas vezes, é excluído pela sociedade, mas merece respeito, é experiente e útil à sociedade. A terceira temática, permitiu-nos compreender as representações sociais que os enfermeiros têm implícitas nas suas práticas de cuidados. Está presente em todas as áreas temáticas que o idoso é uma pessoa dependente, só, abandonado pelas famílias e necessita de atenção, no entanto, há alguma diferença de resultados entre os enfermeiros do Hospital e do Centro de saúde. São mais evidentes nas respostas dos enfermeiros do Centro de Saúde, as questões relacionadas com a solidão, a pobreza e o abandono, por parte das famílias. Esta constatação poderá estar relacionada com o facto destes profissionais terem uma intervenção e um conhecimento na comunidade da situação socio-económica e familiar do idoso. A falta de recursos estruturados para uma intervenção multidisciplinar e multi - institucional, numa perspectiva de intervenção integrada entre as Políticas de Saúde e as Políticas Sociais, provavelmente traz sentimentos de impotência aos enfermeiros do Centro de Saúde, em que as exigências da sua prática faz com que estabeleçam prioridades na área técnica, não lhe sendo permitida disponibilidade para ter um papel de educador, de promotor de parcerias com as famílias e a utilização dos vários recursos da comunidade na melhoria da qualidade de vida do idoso. A representação que o enfermeiro tem do idoso, como sendo uma pessoa dependente e frágil, leva o profissional a fazer uma avaliação e intervenção numa perspectiva reducionista e preconceituosa e não numa perspectiva bio-psico-social e mais positiva

Ageing is a normal process that is not quite well understood, which has contributed for the appearance of some myths and stereotypes concerning to elder people. The aged one isn’t seen anymore as a wisdom being or a story teller but as an “old one" who gives work, who is waiting for death, and isn’t useful. The aged ones are more and more victims of discrimination and stereotypes and there aren’t sufficient support and protection policies to aged ones and also to their families which contributes to isolate them, to put them aside and close them in institutions. These negative aspects only show certain dimensions of what is the ageing, contributing to keep on the stereotypes, myths and the pre-conceived ideas. In this direction, our purpose is to study the Social Representations of the nurses when taking care of the aged one, to determine the influence that the same ones can have in the act of taking care. Relatively to the methodological procedures, we opted for a descriptive and exploratory characters study inserted in a qualitative boarding, once that it allows, knowing and understanding the experiences, attitudes and opinions of the nurses of the study in the context of instalment of cares to the aged one. We carried through half-directive interviews, to eighteen nurses (Health Centre and Hospital). The data analysis had as conceptual board the theory of Social Representations. Three thematic areas had been defined: Concept of aged/oldness/ageing; Experiences that nurses have in relation to the aged one; Attitudes/opinions of the nurses in relation to the cares of nursing to the aged one. On the first and the second areas, the conceptual question is know of what it is to be aged and in what way the nurses experiences can influence this concept. For the majority he/she is a dependent being, alone, unhappy, abandoned, who needs attention, affection, he/she is a child for the second time and excluded by society, but he/she deserves respect, is also experienced and useful to society. The third thematic area allowed us to understand the social representations whose nurses have implicit in their practical care. It is present in all the thematic areas that the aged one is a dependent person, alone, abandoned by families and needs attention. However, there is some difference of results between the Hospital nurses and the Health Centre nurses. Questions related with the solitude, the poverty and the abandonment of the families are more explicit in the answers of the Health Centre nurses. This fact could be related with the fact of these professionals having an intervention and knowledge of the social-economical familiar situation of the aged one. The lack of structuralized resources to a multidiscipline and multi-institutional intervention, in one perspective of an integrated intervention between the health and social policies, probably bring feelings of powerless to the Health Centre nurses, where the demands of their practical experience will establish priorities in their technical area, not being allowed availability of having an educative role, promoter of relationships with families and the use of the several resources of the community in the improvement of the aged one’s quality of life. The representation that nurse has about the aged one, as being a dependent and fragile person, takes him to do an evaluation and intervention in a reductive and pre-concept perspective and non in a bio/psychosocial perspective, but much more positive
