Influência do Diabetes mellitus e da insulinoterapia sobre a osseointegração: avaliação histométrica em coelhos

Abstract

A indicação de implantes dentais osseointegráveis para pacientes com comprometimento sistêmico como o Diabetes Mellitus permanece controversa. O objetivo deste estudo foi avaliar, por meio de parâmetros histométricos, a influência do Diabetes Mellitus e da insulinoterapia sobre a osseointegração. Foram utilizados 34 coelhos adultos da raça Nova Zelândia, os quais foram divididos nos seguintes grupos: grupo controle (C), grupo diabético induzido (D) e grupo diabético tratado com insulina (DTI). A indução do diabetes foi realizada por administração de Aloxana (115 mg/kg). Os coelhos do grupo DTI receberam tratamento com injeção subcutânea de insulina (10 U/ dia) e os demais receberam solução salina (0,9%) pela mesma via. Cada animal recebeu dois implantes para análise histométrica. Após 4, 8 e 12 semanas, os animais foram sacrificados e as peças reduzidas para processamento histológico. Os três grupos não apresentaram diferenças em relação a área óssea (AO). Entretanto, o Grupo C apresentou maior extensão de contato osso/implante (ECOI) nos três períodos experimentais P<0,001 e os Grupos D e DTI não mostraram diferenças estatística em relação a ECOI P= 0,75. Assim, pode-se concluir que o Diabettes Mellitus influenciou negativamente a osseointegração de implantes colocados em tíbias de coelhos e a insulinoterapia não alterou este efeito.

The indication for the placement of osseointegrated dental implants in patients with systemic illness, like diabetes, remains controversial. The purpose of this study was to evaluate, through the histrometric parameters , the influence of Diabetes Mellitus and its treatment with insulin, on osseointegration around implants. Thirty four New Zealand adult rabbits were separated in 3 groups: control group (C), diabetes group (D) and insulin treatment diabetes group (DTI). Aloxana (115mg/kg) was used in diabetes induction. The DTI group received treatment with insulin (10 U/ day) and the others received saline solution (0,9%). After 1 week, implants were placed bilaterally in the tibiae of rabbits. After 4, 8 and 12 weeks, the animals were sacrificed and the implants were analyzed. The bone area didn'tþ show statistically greater values to the D, C e DTI groups. The C group showed statistically greater values bone contact with the surface implant than D and DTI groups in all periods (p<0,001). The groups D and DTI didn't showed any significant differences between each other in any period evaluated for bone implant contact (p=0,765). Thus, it may be concluded that Diabettes Mellitus can negatively influence the osseointegration around implants in tibiae of rabbits and insulin therapy did not altered this effect.
